Required Forms

Peru State College requires that each camper prints off and fills out a Release and Medical Authorization Form. Each camper must bring the completed form to camp. The Release and Medical Authorization Form must be signed by a parent or guardian if the camper is under 18 years of age. Students who are 18 years old or will become 18 years old before the end of the camp/clinic must also sign.

If a camper does not bring a completed Release and Medical Authorization Form to camp, the camper may not be allowed to participate in camp activities.

Campers who register online will be emailed the required forms.
